The Culture Care Connect (CCC) program brings together key streams of suicide prevention planning, coordination and activity across the network regions. These include raising awareness, early intervention, crisis management and aftercare services. This provides a strong and clear pathway between community controlled and mainstream services and acute care settings for clients at risk of or recovering from a suicide attempt, their families, and Communities.

The integration of suicide prevention planning and aftercare services enables an Aboriginal and Torres Strait Islander-led, coordinated approach to reduce the prevalence of self-harm, suicide ideation and suicide in a way that meets the needs and priorities identified by communities.

Aftercare service provides a safe and secure environment, culturally sensitive in a systemic integrated treatment case manage approach fostering a collaborated and coordinated assistance in connecting with various partners. CCCP are crucial for prevention, serving as a dependable and respected avenue for individuals, families and communities to engage in a culturally sensitive discussions about suicide.
